Requesting Mice

In order to supply the requested mice, we will need the following information prior to shipment:

  1. A completed request form, Request for Mice, of shipping information and requested stocks containing:
    • Name of Institution receiving mice
    • Institution's PHS (Public Health Service) assurance #
    • Requesting PI's name, phone, and email
    • Animal facility contact's name, phone, and email
    • Stock(s), sex, age, number of mice requested
    • Requestor's name, phone, and email

  2. A signed Material Transfer Agreement (MTA) for those wishing mice for academic, non-commercial, or non-developmental purposes. Agreements that potentially involve commercial development must be negotiated separately. A sample MTA can be viewed on the web, but the official document must be requested from the Office of Technology Transfer by Patricia Hunsicker. Return the MTA as instructed by the Office of Technology Transfer.

  3. A check for the full amount owed for mice, made out to UT-Battelle, LLC, and referencing the MTA number (found in the top right corner of the official MTA form) must be received before shipment can proceed. The signed MTA/Invoice is your receipt.

Each mouse will cost $50.00 for non-federal users/sponsors, or $42.00 for federal users/sponsors. This cost is based on our Department of Energy requirements for cost recovery, and is subject to adjustment. Checks must be received before shipment can proceed.

    Send check to:
